PLD has partnered with Livez to help you take charge of your health through preventive and annual screenings. By staying proactive with your health, you can earn up to $120 in gift cards.
Rewards are available to a limited number of employees. Each quarter, new opportunities open for employees who have not yet received their rewards, giving everyone a fair chance throughout the year. Once you earn your rewards, you have successfully completed the program.

Livez is your free preventive health screening app. We provide personalized care plans, health self-tests you can do from home, and help you schedule professional screening appointments.
You can easily book appointments with your existing doctors or find new ones in-network directly through the Livez app. All our activities follow validated medical guidelines to ensure you receive the best preventive care.

Level 1 ($20 Gift Card):
Complete all assigned self-tests in the Livez app. These are quick assessments for heart health, anxiety, and more, tailored to your personal profile.
Level 2 ($100 Gift Card):
Complete your entire personalized care plan. This includes attending your professional screenings (like annual exams or blood work) or marking them as “Done” in the app if you completed them on your own.
We’ll notify you by email immediately upon completing each level so you can claim your gift card reward.
We made it easy. You will receive an email and SMS with the link to your gift card. From there, you just select one of the brands you like and redeem it, from hundreds of famous brands.
Good news! If you’ve already completed any self-tests or professional screenings during the current program year, you’ll receive credit for them.
Simply log into the Livez app and mark those specific activities as “Done” to ensure they count toward your Level 2 reward.

Your personal health information will not be disclosed to PLD, insurance companies, or any other third parties. We only share confirmation that you’ve completed a level to facilitate your reward.
