FBIN Wellbeing Program - Questions & Answers

The Fortune Brands Wellbeing Program is designed to help you take charge of your health with preventive and annual screenings and includes a cash reward incentive for U.S full-time associates.

We’ve partnered with Livez for you to simply complete your health screenings virtually and to assist you with appointment scheduling for screenings that require a visit to a physician to earn up to $250 in rewards.

Open to associates only, the cash reward incentive is available for activities completed from December 1, 2025 through November 30, 2026.

Livez is a free preventive health screening app offering personalized care plans, health self-
tests, and professional screening appointments, including an option to easily book your
appointments with your existing in person doctors and providers, or find new ones in-network
and book directly through the Livez app.

All activities follow validated medical guidelines.

To download Livez, visit livez.ai/fbin or search on your app store for “Livez”.

Level 1 ($50):
Complete your first self-test in the Livez app. These are quick and easy screenings from your phone covering areas like heart health, anxiety, or colon cancer.

Level 2 (Additional $75, total $125):
Finish all remaining self-tests in the app. Each test takes just a few minutes and is tailored to your personal health profile.

Level 3 (Additional $125, total $250):
Complete your personalized care plan. Either attend your professional screenings (e.g., mammograms, blood work, vision exams) or mark them as “Done” in the app if you completed them on your own. Only completed or marked-as-done visits count toward Level 3.

You’ll be notified by email when you complete each level and unlock your reward.

For activities completed after December 1, 2025—stay tuned! We are fine-tuning our program behind the scenes and will be announcing our new redemption process soon.

Self-tests are health assessments within the Livez app, covering areas like cardiovascular health, cancer screenings, anxiety, prediabetes, and more.

To earn rewards by reaching Level 3:

– Appointments must be completed, either through Livez or on your own.

– If you booked an appointment outside the app, just tap “Mark as Done” in the app after your appointment is finished.

– If you booked an appointment through Livez, your activity is automatically credited once the visit happens.

Livez customizes your preventive care plan based on your age, gender, race, lifestyle, and personal and family medical history.

It follows clinically validated guidelines from trusted sources like the CDC, USPSTF, and other U.S. preventive health authorities to recommend the screenings and self-tests most relevant to you.

Yes, your data is strictly protected according to HIPAA and AWS SOC2 standards. Using Livez is confidential and your data will not be disclosed to Fortune Brands, insurance companies, or third parties.

While Livez is available to all full-time U.S Fortune Brands associates, their spouses and dependents aged 18+, the $250 screening incentive is only available for Fortune Brands associates to earn.
